Balancing what is desired with what is required, Lifeline’s team of expert Care Managers honor the person while addressing the priorities through a comprehensive senior care plan that creates successful outcomes for all concerned. Empowered with your trust, Lifeline Care Managers do everything necessary to ensure your senior lives the best life possible. Experienced with issues such as medications, physician interaction, memory concerns, behavioral or mood changes, in-home safety, driving impairment, and purposeful activity engagement, Lifeline Care Managers seek to understand the concerns and realities while bringing dignity to every situation. Beginning with a thorough on-site evaluation, Lifeline’s Care Managers determine the feasibility of remaining at home and provide an extensive and tailored activity plan.

Lifeline Care Managers

    •    Are available for phone consult to assist with a variety of concerns regarding changes in physical and cognitive conditions.
    •    Prepare and implement global Health & Wellness Assessments and Recommendations.
    •    Participate in regular in-home visits.
    •    Prepare & implement weekly activity calendars, tailored to your senior’s needs and unique interests.

Dad’s confidence in you from the beginning was inspired by your professional knowledge, your willingness to explain everything to him and Mother and your plan of care to help them maintain a maximized quality of life.
Much of what your planning and vision provided have been providential and preventative: meeting with the insurance agent before they needed the in-home  care, finding someone to help with in their home just before Dad broke his hip, counseling with them during his recuperation and so many phone calls at all hours to allay fears of the unknown.

It is the personal touches which make your service more than just a job well-done.

The caregivers who work with you are well-chosen for their empathy, gentleness, patience and ability to engage clients in meaningful activities. 

Another aspect of your services which we had not anticipated is your participation in his care plan meetings with the staff at the nursing home.  The goals are much more realistic than they might have been without your input and aid in understanding of Dad’s condition.  Your consistent input has impressed upon others that Dad is still a capable adult, and a thinking and feeling human being.  You have helped to maintain his dignity.

Your keeping in touch with us has been so vital.  The phone calls when he is having a good day helped to diminish my frustration on bad days.  Knowing that you are there through the ups and downs and taking advantage of the activities at home made us realize how large a burden you have taken from us.  We are grateful that you have taken a personal interest in Dad.  I only wish that we had found you a few years earlier.
