• Are available for phone consult to assist with a variety of concerns regarding changes in physical and cognitive conditions.

Another aspect of your services which we had not anticipated is your participation in his care plan meetings with the staff at the nursing home. The goals are much more realistic than they might have been without your input and aid in understanding of Dad’s condition. Your consistent input has impressed upon others that Dad is still a capable adult, and a thinking and feeling human being. You have helped to maintain his dignity.
Your keeping in touch with us has been so vital. The phone calls when he is having a good day helped to diminish my frustration on bad days. Knowing that you are there through the ups and downs and taking advantage of the activities at home made us realize how large a burden you have taken from us.
